1.Scope

1.1This Privacy Policy ("Policy") governs ForLife's collection, use, disclosure and retention of personal data in connection with (i) the website forlife.care ("Website"); (ii) enquiries made to ForLife; and (iii) the healthcare programmes ForLife operates with partner clinics ("Partner Clinics") and the services and systems used to operate them ("Programme Services").

1.2This Policy is published under the Information Technology Act, 2000 and the rules made thereunder, and the Digital Personal Data Protection Act, 2023 (together, "Applicable Law").

2.Personal data collected

2.1Enquiries. Where you contact ForLife through the Website, by email, WhatsApp or telephone, or through a lead form on an advertising platform (including Facebook, Instagram and Google), ForLife collects your name, contact details, the programme of interest and the nature of your practice, and any correspondence.

2.2Website. ForLife collects standard technical data and uses cookies and similar technologies, including analytics and advertising tools of providers such as Meta and Google, to measure use of the Website and the performance of its advertising. You may disable cookies in your browser and adjust advertising preferences in your Meta and Google account settings.

2.3Public practice details. ForLife may use practice details a medical practitioner has made publicly available, including on a clinic website or directory, to contact the practitioner regarding the Programme Services. A practitioner may object at any time by email.

3.Purpose

3.1ForLife processes enquiry data only to respond to the enquiry, discuss the programme concerned and assess the performance of the Website and its advertising. ForLife does not sell personal data.

5.Disclosure

5.1ForLife discloses personal data only to (i) service providers that host the Website and support enquiry management and messaging, acting on ForLife's instructions; (ii) the advertising and analytics platforms referred to in Clause 2.2, where you submit a form on that platform or its tools record your visit, subject to that platform's privacy policy; and (iii) governmental or regulatory authorities, where required under Applicable Law.

6.Security

6.1ForLife maintains reasonable security safeguards and restricts access to personal data to personnel who require it for their functions.

7.Retention

7.1Enquiry data is retained for up to three years from the last contact, or longer where you become a customer or Applicable Law so requires. ForLife shall delete it earlier on request.

8.Patient Data

8.1A Partner Clinic's medical practitioner determines a patient's eligibility for a programme and prescribes it. The Partner Clinic refers the patient to ForLife and obtains the patient's consent to share the patient's personal data ("Patient Data") with ForLife and the persons and providers required to operate the programme. For a patient under 18 years of age, the Partner Clinic obtains such consent from a parent or guardian.

8.2ForLife processes Patient Data only to operate the programme prescribed and to keep the patient informed of its progress. Medical decisions, medical records and the management of adverse events remain the responsibility of the Partner Clinic and its medical practitioners.

8.3Patient Data is accessible to the Partner Clinic and its medical practitioners, ForLife personnel, and the suppliers and service providers engaged to operate the programme, each for that purpose alone. ForLife does not use Patient Data for its own marketing and does not sell it. ForLife may use de-identified information to assess and report the performance of its programmes.

8.4Communication with ForLife following referral by a Partner Clinic forms part of the programme. Where a patient withdraws consent during a programme, ForLife may be unable to continue it and shall inform the Partner Clinic.

8.5Patient Data is retained for the duration of the programme and thereafter for the period required under Applicable Law.

9.Partner Clinics and practitioners

9.1With the Partner Clinic's agreement, ForLife may identify the clinic as a partner on the Website and in its online communications. A practitioner's name, photograph and qualifications are displayed only with that practitioner's consent and are removed on request or on termination of the agreement.

9.2Contact details, agreements and invoices of Partner Clinics are retained for the term of the agreement and thereafter as required under Applicable Law.

10.Your rights

10.1You may request access to, correction or erasure of your personal data, withdraw consent, nominate a person to exercise your rights, or raise a grievance, by email to hello@forlife.care.

10.3Requests concerning medical records or a consent obtained by a Partner Clinic shall be referred to that clinic, and ForLife shall assist in their resolution.

11.General

11.1ForLife may amend this Policy at any time.

11.2This Policy is governed by the laws of India. Courts at Bengaluru shall have exclusive jurisdiction.
